If anyone in your home has known or suspected Covid19 exposure, been told to quarantine, traveled anywhere overnight, or had any illness symptoms in the past 2 weeks please call the office for further instructions. Please note our new policies: 1. Upon arrival you must STAY in your car and call 401-231-3138 option 2 leave your child(ren)’s name and the cell number you are using reached at and await our call to register your child. 2. Only one parent / caregiver should come in for the appointment. 3. Siblings should NOT come in with out their own appointment. 4. Anyone 2 and older should wear a non-valve face mask over nose and mouth. 5. Please log in to CHADIS.com and fill out any assigned questionnaires prior to arrival for physicals or medication checks. If you have any questions please call the office at 401-231-3138.
Check out
Please stay in your room until the Medical Assistant comes back in and clears you to leave. The MA will be give you any needed paperwork and make any needed follow-up appointments. Please leave the office immediately once you are released. Do NOT stop at the desk.
